What happened?
CMS published the fiscal year (FY) 2027 ICD-10-CM code set in summer 2026, together with the addenda, tabular and index files, and the ICD-10-CM Official Guidelines for Coding and Reporting for FY 2027. The new code set takes effect on October 1, 2026, the first day of the federal fiscal year.
What changed?
According to industry summaries of the release (ACDIS and AGS Health report the same totals), the FY 2027 update includes:
| Change | Reported count |
|---|---|
| New codes | 190 |
| Deleted codes | 30 |
| Revised code titles | 4 |
Highlights reported across those summaries include:
- Obstetrics — a new subcategory for continuing pregnancy after vanishing twin syndrome (O31.4-), reported as adding 33 codes organized by trimester and fetus.
- Toxic effects — new specificity in category T52 for additional organic solvents, including cycloparaffins and alkenes.
- Neoplasms — more specific codes for certain secondary malignant neoplasm sites.
- Cardiology — expanded cardiomyopathy and inherited arrhythmia classification, as reported by AGS Health.
- Genetic conditions — dedicated codes for several named syndromes that previously required less specific codes.
We are not reproducing code titles from the tabular list. Review the FY 2027 addenda for the exact codes, titles, includes/excludes notes and instructional changes.
Who is affected?
Why this matters to coders and billers
ICD-10-CM codes are validated against the date of service. A claim that reports a deleted code for a date on or after October 1 — or a brand-new code for a date before it — is likely to be rejected or denied. New codes also often add specificity that only appears in the record if providers document it, which is why the annual update is as much a documentation event as a coding one.
Effective date
Practical impact
- Encoders and EHR pick lists must be loaded with the FY 2027 tables; stale favorites lists are a common source of deleted-code errors.
- Inpatient claims use the discharge date to determine the code set; outpatient claims use the date of service.
- MS-DRG assignment for FY 2027 discharges uses the FY 2027 grouper released with the IPPS final rule — see our FY 2027 IPPS final rule explainer.
- Held or late claims spanning the transition need the code set that matches each date of service.
